Introduction to NYS HIV Case Management
This course will provide participants from COBRA and AIDS Institute grant-funded programs with the basic understanding of the case management process.
Topics covered in this course include:
- Review and comparison of case management models.
- Conducting an effective intake.
- Components and relationship between steps of the case management process.
- Case coordination with other service providers.
- Service Monitoring and Follow-up.
Prerequisite
Although there is no prerequisite for this course, we recommend that participants have previous training in HIV confidentiality and basic HIV/AIDS medical information.
Audience
New case management staff and health and human service providers including:
- Direct-service staff.
- Supervisors.
- Program Directors.
Cost
Free and open to the public.

This course will prepare participants from COBRA and AIDS Institute grant-funded programs with the tools necessary to conduct thorough, effective and meaningful HIV/AIDS comprehensive needs assessments and develop successful service plans.

- Interviewing and Documenting at the Same Time
- Questioning & Developing Rapport
- Assessment
- Service Plan
- Goals, Objectives and Tasks
- Developing a Successful Service Plan
